Tesla remote operators keep crashing its ‘Robotaxis,’ data shows

15 October 2026 at 17:39

Tesla’s latest crash reports to federal regulators add two more “Robotaxi” crashes where a remote operator, not the car’s autonomous system, was driving when it hit something.

That brings it to five in the data now. And every remote takeover that has shown up in a Tesla crash report has ended in a collision.

Tesla confirms Autopilot/FSD was ‘engaged’ when car crossed a median and killed a man

15 October 2026 at 15:03

A Tesla crossed the grass median of Route 55 in New Jersey, hit a tree, slid into oncoming traffic, and struck an SUV head-on. The SUV’s driver, 68-year-old Phillip Eastman, was killed.

Police reported it as a driver who lost control. What they didn’t say, and likely didn’t know, is that Tesla’s own report to federal regulators says the car’s driver-assist system was engaged. This is part of an ongoing Electrek investigation matching reported crashes to Tesla’s redacted NHTSA data.

NHTSA weighs probe into claims Tesla Robotaxis speed in school zones

12 October 2026 at 14:24

A new defect petition accuses Tesla’s Robotaxis of speeding through active school zones, and NHTSA has opened a file to decide whether to investigate. The agency puts the affected fleet at about 1,000 vehicles, Model Y and Cybercab.

It’s the third federal action aimed at Tesla’s driverless fleet in about five weeks, after an audit query and a Special Order over how the Cybercab was certified.

Tesla Cybercab loses its entire door panel in a Philadelphia crash

7 October 2026 at 19:44

A Tesla Cybercab was involved in a crash in Philadelphia this week, and it came out of it missing the entire outer panel of its driver-side door.

We don’t know who was at fault or whether Tesla’s software was driving. But Tesla has no permit to run automated vehicles in Pennsylvania, and that narrows down what this car was doing on Broad Street.

NHTSA investigates comma.ai’s openpilot over 5 crashes that killed 3

24 September 2026 at 20:12

NHTSA has opened a formal investigation into comma.ai’s aftermarket driver assistance devices after five crashes where cars running openpilot plowed into stopped or slow-moving vehicles in their own lane. Two of those crashes killed three people.

The probe covers an estimated 30,000 comma three, comma 3X, and comma four devices. It’s the biggest regulatory problem for George Hotz’s startup since NHTSA pushed it to cancel its first product back in 2016.

NHTSA orders Tesla to prove its Cybercab is legal to sell, under oath

15 September 2026 at 22:00

NHTSA has ordered Tesla to prove that its Cybercab is even legal to sell. The agency issued a formal Special Order on September 10 demanding that the company show, under oath, how a vehicle with no steering wheel, no pedals, and no mirrors complies with federal safety standards written for human drivers.

Tesla has until September 30 to respond. Failing to answer fully and truthfully carries penalties of up to $139 million.

Tesla’s record month: 4 fatal Autopilot and Full Self-Driving crashes

15 September 2026 at 15:05

July 2026 was the worst month on record for Tesla driver-assist crashes. Tesla reported 236 of them to federal regulators, the most it has ever filed in a single month.

Four of those crashes killed someone. In every one, Tesla’s own data says Autopilot or “Full Self-Driving” was engaged, and in every one, the public was never told the system was involved. This is the latest from our ongoing investigation matching Tesla’s redacted NHTSA reports to real, named crashes.

Tesla Autopilot was on in I-35 crash that killed its driver

10 September 2026 at 20:18

A 2024 Tesla Model Y ran off Interstate 35 in rural Missouri, struck a guardrail, and killed its driver, a 64-year-old woman from Raymore. Troopers reported a single-vehicle wreck and said the car “went off the road.”

What they didn’t say, and likely didn’t know, is that Tesla’s own report to federal regulators says the car’s driver-assist system was engaged. This is part of an ongoing Electrek investigation matching reported crashes to Tesla’s redacted NHTSA data.

Congressman presses DOT on Tesla FSD after 43 sleeping-driver videos

10 September 2026 at 16:48

A US congressman is demanding the Department of Transportation explain what it’s doing about Tesla’s Autopilot and Full Self-Driving after an NBC News investigation surfaced dozens of videos of drivers apparently asleep at the wheel.

Rep. Raja Krishnamoorthi gave Transportation Secretary Sean Duffy until September 30 to answer five questions about the systems, which Tesla itself says “require active driver supervision and are not autonomous driving systems.”

Data confirms Autopilot/FSD was active in crash that killed a mother, passerby saved her baby

9 September 2026 at 15:12

A 2021 Tesla Model Y left an Alabama highway, hit two trees, and caught fire, killing its driver, 29-year-old Kayleigh Page. Her 7-month-old daughter was pulled from the burning car by a stranger who saw it happen.

Troopers reported it as a single-vehicle wreck. What they didn’t say, and likely didn’t know, is that Tesla’s own data shows the car’s driver-assist system was engaged. This is part of an ongoing Electrek investigation matching reported crashes to Tesla’s redacted NHTSA data.

A Tesla ran a stop sign and killed a man, record confirms Full Self-Driving/Autopilot was on

7 September 2026 at 19:41

A Tesla Model 3 ran a stop sign in Buena Vista Township, New Jersey, and hit a Honda Civic turning left through the intersection. The Civic’s driver, 82-year-old Stephen Field, was killed.

Police reported it as a car that failed to stop. What no one reported is that Tesla’s own data says the car’s driver-assist system was engaged. This is part of an ongoing Electrek investigation matching reported crashes to Tesla’s redacted NHTSA data.

Tesla Cybercab is already under NHTSA investigation after launch

4 September 2026 at 13:46

Tesla’s Cybercab is already under federal investigation. NHTSA opened an audit into how Tesla certified the vehicle the same day the company started putting passengers in it in Austin.

The probe targets the exact thing that makes the Cybercab unusual: it has no steering wheel, no pedals, and no mirrors, and Tesla certified it as road-legal anyway.

A Tesla killed a motorcyclist on Autopilot/FSD, Tesla’s own data confirms

3 September 2026 at 17:05

A Tesla Model X struck and killed a motorcyclist at an intersection in Moraine last year and the crash went mostly unnoticed.

But Tesla’s own report to federal regulators says the vehicle was a Model X with its driver-assist system, Autopilot or Full Self-Driving (Tesla hides which one) engaged. This is part of an ongoing Electrek investigation matching reported crashes to Tesla’s redacted NHTSA data — and it’s the clearest example yet of how completely these crashes can vanish from the public record.

NHTSA probes 1.2M Tesla Model 3, Model Y over suspension failures

31 July 2026 at 16:57

The US National Highway Traffic Safety Administration has opened an investigation into roughly 1.2 million Tesla vehicles after receiving 156 complaints of a front suspension component detaching while driving.

The probe covers 2018-2020 Model 3 and 2021-2023 Model Y vehicles, and it centers on the front lower lateral link separating from the car.

NHTSA denies Tesla door-release defect petition, opens new door rules

24 July 2026 at 14:16

NHTSA has denied a petition asking it to open a defect investigation into the hidden manual door releases on the Tesla Model 3. But in the same decision, the agency agreed to start writing new door-safety rules for the entire auto industry.

The denial, published today, covers 179,031 model year 2022 Model 3 vehicles. NHTSA said it found exactly one complaint matching the petition’s allegation — and it came from the same owner who filed it.

Tesla Autopilot and FSD crashes hit record 207 in a single month

22 July 2026 at 18:02

Tesla reported 207 crashes involving its driver-assist systems in a single month — the most it has ever logged. The figure comes from Tesla’s own crash reports to the National Highway Traffic Safety Administration (NHTSA).

The record month lands as Tesla pushes Full Self-Driving harder than ever, and as the company continues to redact nearly every detail of what actually happened in these crashes.

NHTSA targets Tesla’s ‘Radar Saves Us’ document in FSD crash probe

21 July 2026 at 21:37

NHTSA is now demanding a Tesla internal document titled “Radar Saves Us” as part of its investigation into “Full Self-Driving” crashes in low-visibility conditions.

The request is buried in a new 24-part information demand sent to Tesla on July 2, and it points the probe straight at the decision that started all of this: Tesla removing radar from its cars.

Tesla remote operator crashed a ‘Robotaxi’ in Houston, NHTSA data shows

20 July 2026 at 14:15

Tesla has filed four new “Robotaxi” crash reports with NHTSA, and the most revealing one involves a Tesla remote operator driving the vehicle into a tree stump in Houston.

It’s the third crash Tesla has now reported involving a human operating the car remotely caused the collision — and the first one Tesla has formally coded as remote operation in its filing.

Tesla admits FSD was on in fatal Texas crash, blames driver for ‘overriding’ it

23 June 2026 at 16:04

Tesla now confirms that its self-driving system was engaged when a Model 3 left a residential road in Katy, Texas, and killed a 76-year-old woman inside her home — but the company says the driver overrode it by pressing the accelerator to 100%.

Everything points to a pedal misapplication while the car was under “Full Self-Driving.” Does that mean Tesla is not at fault?

NHTSA probes fatal Tesla crash into Texas home that killed woman

22 June 2026 at 21:56

Federal safety regulators are now investigating the fatal Tesla crash in Katy, Texas, where a Model 3 left a residential road, tore through a brick home, and killed a 76-year-old woman inside.

The National Highway Traffic Safety Administration said Monday it is examining the June 20 crash, after the driver told deputies his Tesla was on Autopilot at the moment of impact.
